In the event of an illness, injury, or medical emergency involving my child during Vacation Bible School, I authorize the adult leaders, volunteers, or representatives of Salt and Light to seek emergency medical care for my child if I cannot be reached in a timely manner.
I understand that reasonable efforts will be made to contact me or the emergency contact listed on this form before treatment is obtained when possible. I authorize licensed medical professionals to provide emergency medical treatment deemed necessary for the health and safety of my child.
I understand that I am responsible for any medical expenses incurred as a result of illness, injury, or emergency treatment.
I give permission for my child to participate in Vacation Bible School activities sponsored by Salt and Light, including but not limited to Bible lessons, worship, games, crafts, snacks, indoor activities, outdoor activities, and group events.
I understand that participation in children’s activities may involve normal risks, including but not limited to slips, falls, minor injuries, allergic reactions, or other unexpected incidents. I acknowledge these risks and give permission for my child to participate.
To the fullest extent permitted by law, I agree not to hold Salt and Light, its pastors, staff, officers, volunteers, members, agents, or representatives responsible for ordinary accidental injury, illness, loss, or damage that may occur while my child participates in Vacation Bible School activities.
This release does not apply to conduct that cannot legally be released under applicable law.
Any adult bringing a child with special needs will have to go through a background check (paid for by us) and attend with their child. We will have scheduled activities as well as an area to calm down/sensory area. We will reach out for more details following your sign up.
